This position will work in partnership to provide targeted and opportunistic health screening (Including screening for Chronic Conditions), health promotion, follow up and education. The position will provide support and advocacy for all Aboriginal and Torres Strait Islander in-patients within all hospitals that are located within the Northern Sydney Local Health District. Along with assist to improve the sensitivity and understanding of all health care providers with respect to Aboriginal cultural beliefs and other issues that affect Aboriginal and Torres Strait Islander people.
This position will also assist in the intervention, prevention and referral of Aboriginal clients, families and carers to Clinical Nurse Consultants, Nurse Practitioner and General Practitioners for assessment, case review and development of clinical management plans for those who are identified as being at risk of developing or with established chronic disease, to access health services.
